Acenda Integrated Health Launches Demonstration Pilot Program, LifeSet

Beginning October 1, Acenda Integrated Health (Acenda) will be participating in a demonstration pilot program that provides intensive, supportive services for at-risk youth transitioning from New Jersey’s foster care system.

Acenda is partnering with the New Jersey Department of Children and Families’ Office of Adolescent Services and Youth Villages, an agency from the State of Tennessee that developed the LifeSet model, to anticipate, address and meet the service needs of participating youth.

“Many teens and young adults rely on our programs for support and guidance as they take the next steps in shaping their adult lives,” said Dr. Anthony DiFabio, President & CEO, Acenda. “We are honored to have been selected to participate in this demonstration pilot program to help build a stable foundation for our community’s youth.”

For youth and young adults leaving the foster care, juvenile justice, and mental health systems, LifeSet will provide guidance to help them learn – or sharpen – the life skills they need to manage adulthood. The goal is for them to maintain stable and suitable housing, remain free from legal involvement, participate in an educational/vocational program, and become thriving citizens.

Erin Klein, Program Director for Acenda’s Life Set Program, added “The program will provide direction to these young adults, bringing the individual’s support systems together to facilitate a positive and productive transition to the adult world. When we work as a team, the young people we service learn the value of interdependence and personal accomplishment.”

Youth Villages’ outcomes data indicate that this program model significantly reduces the risk of homelessness, poverty, and intimate partner violence among participating young adults.
